Primary Job Duties:
                                       Therapeutic Recreational Specialist / Discharge Coordinator
Activity groups help people reduce depression, stress, and anxiety; recover basic physical and mental abilities; build confidence; and socialize effectively. Activity groups are effective in helping people with disabilities integrate into the community by teaching them how to use community resources and recreational activities, such as arts and crafts, games, dance, or sports, to develop new social skills and coping skills.
Minimal Qualifications:
·         Certification as a Therapeutic Recreation Specialist (CTRS) or Registered Therapeutic Recreator (RTR) preferred.
Job Responsibilities & Performance Standards: 
  • Conducts activity groups to improve individual’s mental, physical, cognitive and social well-being document accordingly into the electronic patient chart.
  • Confers with members of treatment team to plan and evaluate activity programs. 
  • Develops treatment plan to meet needs of individual, based on needs assessment, individual’s interests and objectives of activity. 
  • Encourages individuals with limitations to acquire new skills and get involved in health-promoting life skills and leisure activities, such as sports, games, music, and arts and crafts.
  • Instructs individuals in activities and techniques, such as sports, gardening, dance, music, art, relaxation techniques, or other similar activities designed to meet their specific physical or psychological needs. 
  • Observes, analyzes, and records individual’s participation, reactions, and progress during treatment sessions, modifying treatment programs as needed. 
  • Obtains information from medical records, medical staff, family members and the individuals themselves to assess individual’s capabilities, needs and interests. 
  • Plans, organizes, directs and participates in treatment programs and activities to facilitate individual’s rehabilitation, help them integrate into the community and prevent further medical problems. 
  • Prepares and submits reports and charts to treatment team to reflect individual’s reactions and evidence of progress or regression.
  • Complete functional assessment on each patient upon admission in a timely manner.
  • Develop treatment plan goals and enter these onto the interdisciplinary treatment plan within a timely manner.
  • Assist social services with discharge coordination as needed and directed by LCSW.
  • Assist with court document preparation as needed.
  • Assist with scheduling and facilitation of court psychiatric evaluations as needed.
  • Facilitation of mental health court via video teleconference as needed. 
  • Other duties as needed and assigned.
